Forms and questionnaires

Patient Watch includes validated patient-reported outcome measures (PROMs). Some are simple pain or function scales; others, such as the Oxford Knee Score or EQ-5D-5L, need built-in calculators so scores are computed correctly for you and your patients.

What is included today

Patient Watch ships with a library of validated forms used in orthopaedics, pain, and general health research. Your template picks which ones patients see and when.

  • Pain and function scales (for example VAS pain, PSFS, QuickDASH)
  • Condition-specific PROMs (for example KOOS, WOMAC, Oxford Knee Score)
  • Generic health measures (for example EQ-5D-5L)
  • Compliance and treatment-specific forms your clinic already uses

Forms that need calculators

Some PROMs are not just a sum of tick boxes. They need a scoring function that matches the published methodology - otherwise exports and charts are misleading.

Oxford Knee Score

Twelve items scored automatically to a single knee function score. Widely used after knee arthroplasty.

EQ-5D-5L

Health-related quality of life with index and visual analogue score calculated from patient answers.

KOOS, WOMAC, and similar

Multi-domain questionnaires where subscales and totals must follow the published scoring rules.

We maintain calculators in the platform so patients only answer questions and clinicians see the correct score in the diary and exports.

Need a questionnaire that is not in your template picker yet? Tell us the name (for example FAOS, MOXFQ, or a clinic-specific form) and we will confirm whether it is already available, in progress, or needs to be added.
